I don't necessarily think it matters that Ethan in particular survived but I think it was important for the later reveal that someone from Brandon's group survived to pass on the information to Ned. I think that's part of it. The other half though is Ethan, because he's Brandon's squire, would know what was said to Brandon that got him all hot and bothered. We have no idea what made Brandon go after Rhaegar, but Ethan would've known. The difference between Howland surviving and Ethan surviving is that Howland wouldn't necessarily have that backstory information of what happen between the tourney and the death of the Starks. Sure, Howland knows a lot, but he was not present for all the shite that went down in the Vale (where I think Brandon was returning from) or the message that Brandon got, and finally being the only possible link to how Ned knew to go south in order to find his sister. Ethan of all the character had info directly connected to the cause of the rebellion. Howland may have some, but it would be second or third hand news.

Another idea is Aerys left him alive to give a firsthand report of the deaths of the other northerners. The only problem with this scenario is that Ethan didn't get released until the rebels took King's Landing. Ethan wasn't sent off to be a messenger while Aerys was alive. He was put in the cells and stayed there. I think your other idea is a better one, that someone asked for him to be spared, just like Dontos. And it would be brilliant if it turned out to be Jaime that asked for Ethan to be spared.

What the purpose would be though?... So that Ned could find his sister I would imagine. Varys, if he's telling the truth, just wants a stable and prosperous kingdom. Returning Lyanna to her family would stop any further aggression for the Rebels. Or Varys knows summat we don't know. As usual.
